The Agency is a community-based program that provides intermittent skilled professional services and related clinical care to the patients who are in transitioning from the inpatient experience to the home environment. All care is provided under the supervision of the patient’s physician.

After initiation of the services our clinicians make arrangements to provide patients with all necessary medical equipment and supplies. Training on the use, maintenance and cleaning of equipment is provided as necessary.
