How It Works
A program is carefully designed for each patient and family to assist in selecting the best health care team for each case. The relationship between the patient and the home care professional is closely monitored to ensure continuing satisfaction and care.
Here is how it works:
- The Care Team Manager performs a client assessment to determine if the patient qualifies for home health care services.
- The clinician consults with the physician to determine the patient's individual home care needs.
- A detailed plan of care is developed based on the diagnosis, care needs, prognosis, family support, home environment, and community resources available.
- A financial estimate is prepared for the proposed services, and a complete insurance investigation is performed to determine applicable benefits.
- A team of health care and in-home support staff is selected and oriented to the case, and the total home care program is implemented.
- A clinical supervisor makes periodic visits to the home to monitor treatment and review the care plan.
- The physician is continually consulted and provided updates on patient progress throughout the course of treatment, to determine if changes are needed.
